The thoughts are the opposite of who you are — that's why they hurt. Harm coming to people you love. Contamination that no amount of washing settles. The blasphemous, the violent, the sexual intrusions that make you wonder what's wrong with you. And the rituals — checking, counting, confessing, silently praying, googling for reassurance at 1am — that buy twenty minutes of relief and then raise the rent.

The short answer: OCD is very treatable, but it's the one condition on this site where picking the wrong therapy actively backfires. The treatment that works is ERP — exposure and response prevention →, with medication (usually an SSRI, often at higher doses than for depression) as a strong partner.¹ Generic talk therapy built on reassurance performs the compulsion with you. This page exists mostly to stop you from spending a year there.

The scarcity problem, stated plainly

Most California therapists are good people who are not trained in ERP — and OCD is common enough (roughly 1 in 40 adults over a lifetime¹) that the mismatch is a public-health-sized gap. People routinely spend years in supportive therapy feeling understood while the OCD grows. So the search is specialty-first: screen with "What's your ERP training, and how much of your caseload is OCD?" — the full screening script is here →. Telehealth widens the pool statewide (the one rule →), and the IOCDF directory plus our ERP filter both mark the training.

What treatment costs in California

ERP is billed as ordinary psychotherapy: $0 on Medi-Cal (the routes →), $0–40 copays in-network — and because ERP-trained clinicians are scarce, the 10-day rule's out-of-network clause → matters extra: if your plan can't produce an ERP-trained clinician in time, it must cover one out-of-network at in-network cost. Ask for exactly that, in those words. Cash runs the standard ladder (costs →); intensive programs exist for severe cases.

While you search: the one free move

Stop asking for reassurance — from Google, from loved ones, from forums. It's the hardest free thing you can do and the most ERP-aligned: every unanswered "but what if" is a rep. Family members: the loving move is warmth without the certainty ritual — "I love you, and I'm not going to answer the checking question."

Q&A

Q: What is the best treatment for OCD in California? A: ERP (exposure and response prevention), often combined with SSRI medication — and specifically not reassurance-based talk therapy, which can reinforce OCD. Screen therapists for ERP training; if your insurer's network lacks one, California's timely-access law requires out-of-network coverage at in-network cost.
